Understanding Episodic Migraine
In episodic migraine, the nervous system is generally able to achieve long-term stability. Migrainous symptom clusters typically occur only a few times per year. Between these events, patients often feel completely normal, with their neural pathways returning to a pristine, rested state.
However, when an attack begins, neurons that were previously stable abruptly escalate into a state of hyper-excitability and neurogenic inflammation along select pathways. This rapid shift produces the classic symptoms of a migraine attack: intense light and sound sensitivity, nausea, vomiting, and severe, throbbing headache.
Aborting the Attack
Because the nervous system is otherwise stable, episodic attacks can often be successfully aborted if caught early. Medications such as triptans, when taken at the very onset of symptoms, can halt the inflammatory cascade and return the nervous system to its baseline.
