Migraine
Chronic & Transformed Migraine
When episodic attacks evolve into a relentless, fluctuating neurological dysfunction.
What is Transformed Migraine?
Chronic, or transformed migraine, is defined as a variably fluctuating neurological dysfunction represented by two or more migrainous symptoms present for more than 10 days per month.
Unlike episodic migraine where the nervous system resets between attacks, repeated migraine activity causes neurons to fail to return to their baseline state. This chronic hyperexcitability involves many interconnected pathways throughout the brain and brainstem, producing dull, relentless, and overlapping symptoms.
The Symptom Profile
Because so many pathways are involved, the symptoms are far-reaching. Patients often experience not just head pain, but profound anxiety and depression, irritable bowel symptoms, sleep disturbances, and widespread pain such as fibromyalgia. Furthermore, these chronically inflamed pathways typically show a poorer response to standard abortive medications like triptans.
Our Multimodal Approach
Treating transformed migraine requires down-regulating the entire central nervous system. At Creekside Physical Medicine, we use a comprehensive, layered approach to break the cycle of chronic pain.
- Physical and manual therapy to address structural tension
- Deep tissue work to release myofascial holding patterns
- Biofeedback and nervous system regulation techniques
- Advanced medication management
- Peripheral nerve blocks to interrupt pain signaling
- BOTOX® for chronic migraine protocol
